Access to Medical Records

     

ACCESS TO HEALTH RECORDS UNDER THE DATA PROTECTION ACT

The Data Protection Act 2018 gives every living person, or their authorised representative, the right to apply for access to their health records.

  • Any request for access to health records should be made in writing using the ‘Patient Authority Consent Form’ which can be obtained from reception.

  • There will be no charge for patients to receive a copy of their records under GDPR legislation introduced on 25 May 2018 unless the subject access requested is deemed to be ‘excessive’.

  • An access to health records request will be complied with within 28 days.

  • Copies of health records should be collected in person from the surgery; photographic ID must be produced. In exceptional circumstances records may be collected by a third party but only with the prior agreement of the Practice Manager.

  • If a patient feels that the information recorded on their health record is incorrect then they should contact the surgery to discuss the situation.
