Travel Information

TRAVEL VACCINATIONS

Travel vaccination is not included in the services provided by the NHS and is therefore a chargeable service.

Most travel vaccines are ordered on a private prescription and will incur a charge over and above the normal prescription charge.

A consultation/administration fee will also be charged.  This must be paid prior to receiving your prescription:

Adult - £25

Child (under 18) - £10

Booster vaccination - £10

If you require any vaccinations relating to foreign travel please downlaod and complete the form below and return it to reception.

If a prescription is required it will be available to collect 5 days after receipt of the completed form.

You then need to make an appointment with the Practice Nurse to have your vaccination/s administered.

Please allow plenty of time for this process.  Vaccines have to be ordered as they are not a stock item and it is important to receive your vaccinations at least 6-8 weeks prior to travelling to ensure they are effective.

Travel Health Questionnaire

Please download the travel questionnaire using the link below.  Return the completed form to reception.
