TO ORDER REPEAT PRESCRIPTIONS

Repeat prescriptions must now be ordered via our new online service.  If you have already registered for the new service and have received your log in details from the surgery please go to http://www.patient-services.co.uk  to order your medication.

If you have not yet registered for the new service please download the form below and return it to the Surgery as soon as possible.  UNTIL FURTHER NOTICE, REGISTRATION FORMS CAN BE EMAILED TO OUR DEDICATED EMAIL ADDRESS* RECEPTION.Z00471@GP.HSCNI.NET

Repeat prescriptions can also be ordered by leaving your completed green slip or written request into the box at the surgery entrance.

  Patients with no acess to the internet can request repeat prescriptions via our automated voicemail system by dialling the usual number on 028 4062 3303 and selecting the option for repeat prescriptions

This service is only for repeat medication ie medication that is listed on the right hand side of your prescriptions.  Requests for acute medication must be made by telephoning the surgery.

If you require a prescription for an acute condition please telephone the Surgery and leave the details of your condition and your prescription request with the receptionist.

In line with DoH guidance, prescriptions for antibiotics will not be issued over the telephone; patients are required to see a GP before an antibiotic can be prescribed.

If you require an antibiotic for a suspected UTI please bring a sample of urine to the Treatment Room for the Nurses to test.  If necessary, a prescription will then be issued.

Acute prescription requests can not be taken after 4.30pm .
