Patients wishing to register with the surgery must indicate which practice they want to join.

NB:
- the form must be completed in full and signed and dated
- the completed form must be returned to the surgery
- patients completing form HS22X must produce photographic ID
- non EU nationals must produce their visa
- write the name of the practice with which you wish to register on the top of the form
Because it may be some time before we receive your previous medical notes, it is important that each new patient registering completes a health questionnaire. This should be returned to the surgery with your registration form:
